    Our patient’s time is valuable to us; therefore we make every effort to see you at your scheduled appointment time.

    It helps us to provide you with the best service if you:

    1. Confirm your appointment the day before.
    2. Let us know if you are running late.

    Our office will assure you the same courtesy.

    Appointment changes with advanced notice (24 hours) allows us to fill the Doctor’s and Hygiene schedules with patients who may be waiting to be seen at an earlier time. We appreciate your cooperation in helping us continue to deliver prompt, courteous service to all of our patients.

    First time patients should plan to arrive 15 minutes prior to appointment time in order to complete a patient registration form. New patients can also get new patient forms from the web site Info@angellsdentistry.com and complete prior to appointment:

    1. Call our Office to confirm your appointment one business day prior to your appointment.
    2. Should the need arise to cancel or change your appointment, please call the office between the hours of 11:30 am and 7:00 pm Monday & Thursday and 6:30 am to 4:00pm Tuesday & Wednesday and 9:00am and 12:00pm Fridays  Our office requires a 24 hour notice of cancellation. Patients who repeatedly cancel their appointment without notice or simply do not show for their schedule appointment will be charged a fee. Our office phone number is (248) 362-4330.
    3. Complete the enclosed registration, medical history forms and bring them with you to your initial appointment.  Our staff will answer any questions you may have pertaining to the requested information.   Please bring your insurance cards, list of all prescriptions and over the counter medications that you are currently taking. Any required co-payments, pertinent medical and surgical history information and  x-rays.
    4. Bring in all insurance information for dental.  This information should include the policy holder’s name, policy number.  This information will improve our ability to communicate  with your insurance company to assist you in receiving your insurance benefits.
    5. Please bring copies of any recent  dental x-rays full set or panorex taken in the last 3 to 5 years, bite wings taken within the last year.
    6. Patients are required to pay for their office visits, Insurance Co-pays and deductible amounts at the time of service.  We accept cash, personal check or major credit card for payment.  Services covered by Insurance will be billed to the Insurance Company providing we have complete billing information.
